The Life Essentials Assessment Framework (LEAF) is a single assessment tool for older adults.  It provides a unique way to understand quality of life across seven key life domains of: Health, Daily Living, Social Contact, feeling Valued, Enjoyment, Choice and Control, Safety and Security.

 

Developed by Age UK Wakefield District in partnership with older people,  our staff teams and academic partners, LEAF-7 offers you:

 

• A truly person-centred approach – LEAF-7 uses a guided conversation format with built-in goal setting function. This  format facilitates productive discussions between client and support worker, ensuring truly co-created solutions for the client – Read here about How it Works.

 

• A validated measure of quality of life – Assuring you that the impact of the your service on the quality of life of your service users can be reliably measured and reported to funders. Read more about Why it Works.
